We see a glimpse of how protective Sam could be in Need, how far he would have gone to heal Dean is something that needed far more exploration.
This story, how it reveals the ruthless viciousness of the Winchester's lives and psyches is far more satisfactory and was an amazing, believable scenario, very much in character and a very satisfactory coda to Dead Man's Blood.
